Enhancing Performance as Collaborators in Care Presentation

 Slide 2: Hello, I am _____, a practice leader. Today’s presentation will be based on the ways that can be used to improve and increase interprofessional collaboration for a better patient experience. Collaboration between the healthcare teams is beneficial for the patients and the healthcare organizations as they ensure health for all populations and enhance patient outcomes with the reduced cost of healthcare (Bendowska & Baum, 2023). The collaborative environment of healthcare promotes shared decision-making, effective communication, fair division of roles and responsibilities, efficient use of healthcare technology, and reduction in errors which will all lead to the betterment of the organization and the patients along with the well-being of the patients (Montori et al., 2022). 

This presentation will also discuss the educational services that can be implemented to improve the knowledge of the population regarding healthcare and the ways new strategies are being implemented to generate positive patient results along with the satisfaction of staff (Bhattad & Pacifico, 2022). Moreover, newly implemented strategies will be evaluated based on their outcomes and the ethical considerations of the change being implemented. Further improvements will be based on the results of the evaluation to understand its effectiveness.

Interprofessional Collaboration in an Evidence-Based Practice

 Slide 3: Practices based on evidence are integrated into the best available scientifically proven information that can be used in the healthcare decision-making process for better patient outcomes (Lehane et al., 2019). The decisions regarding the best quality of health for the patients depend on the collaborative opinion of the interprofessional and the shared experience of the interdisciplinary team to ensure the health and safety of the population along with the skill enhancement of the healthcare providers (Bouchez et al., 2023). This cooperation can smoothly identify practice gaps and introduce new policies that are implemented to provide the best quality of healthcare services to the population. The steps that can upgrade the evidence-based practice integrated with interprofessional collaboration are given below; 

Establish Collaborative Goals and Objectives

Identifying and establishing a shared goal can be beneficial for the effective involvement of evidence-based decision-making in the healthcare services provided to patients (Montori et al., 2022). The goals should be patient-centered involving the availability of the resources to the population along with improved and competent healthcare services for the vulnerable groups or areas (Ahmed et al., 2022). The objectives should be achievable and measurable and can implemented and evaluated with great care and efficiency for the better cause. 

Effective Communication Strategies

Clear and transparent communication between the team members is of great significance for the accurate sharing of information and a better understanding of the goals and objectives (Nadkarni et al., 2021). Advanced communication and information-sharing strategies need to be implemented to ensure effective results and enhanced patient well-being (Albright et al., 2021). Accurate communication leads to accurate and efficient healthcare results which are the main focus of interprofessional collaborations and healthcare facilities. 

Interprofessional Training Programs

To generate effective health outcomes it is necessary to provide education and training to the healthcare professionals regarding the roles and responsibilities of team collaboration (Baek et al., 2023). These training programs will guide them in sharing opinions that are suitable for the population and the effectiveness of evidence-based practices in efficient recognition and response to deteriorating patient conditions without causing any health disparities.

Implement Evidence-Based Decision-Making Process

Decision-making regarding the health of the patients serves a vital function in improving the collaboration of interprofessional team members and the success of patient treatment (Geese & Schmitt, 2023). The process of taking measures for the betterment of the patients includes great expertise and scientifically evaluated data which can be authorized through the collective opinion of the team.

Planning Stages 

 Slide 4: Proper planning is required for the improvement of interprofessional collaboration regarding the practices based on evidence. A few stages of the planning are given below;

  • Determination of lackings in the collaboration between inter-professionals in evidence-based practice and identify goals that are suitable for patient care (Degu et al., 2023).
  • Design and develop strategies to rectify the knowledge gap for positive clinical results like enhanced communication, use of technology, and so on (Blanchard, 2023).
  • Implement strategies that can be accepted by all the disciplines and healthcare departments effectively (Cassidy et al., 2021).
  • Evaluation of the implemented strategies can be done by the patient’s feedback and by assessing the skill enhancement of the healthcare providers (Berger et al., 2020).
  • The changes in the strategic planning should be done through the accurate evaluation of the practices. These changes need to be in alignment with the patient’s healthcare requirements and the competencies of the professionals to ensure the sustainability and effectiveness of the change.

Overview of Enhancement of Evidence-Based Practices

 Slide 5: The evidence-based practices can be enhanced by creating an environment that promotes teamwork and shared decision-making (Mabona et al., 2022). Continuous learning programs and the availability of evidence to implement evidence-based practices effectively are needed (Dagne & Beshah, 2021). The accurate data keeping and sharing along with the self-engagement of the patients serve as a fundamental process of improving evidence-based practices. Overall these practices help in the planning and implementation of healthcare strategies that are effective in improving evidence-based practices.

Educational Services and Resources

Slide 6: Education about healthcare management and monitoring is essential for the population to ensure their health improvement and timely intervention of healthcare providers (Bhati, 2023). The awareness campaigns along with the training programs help management plans regarding various chronic diseases the people a lot in understanding their health conditions, precautionary measures, and the seriousness of the instructions given to the patients (Chimezie, 2023). Proper implications of the health measures will produce elevated health effectiveness. A few educational services and resources are given below;

Education Of Self-Management

Self-management especially in older adults is of great importance due to their high clinical care demands and health disparity risks (Rony et al., 2024). The assessment and analysis of temperature, blood pressure, blood glucose level, and so on help them and the healthcare professionals in better understating and management of the improvement in the health of patients and identification of any concerned area for the change of policy.

Dietary Consultation Programs

Many patients do not understand the importance of the diet they need to have for their particular disease diabetes patients are mostly unaware of the diabetic diet which results in uncontrollable diabetes before and after meals (Gray & Threlkeld, 2024). Proper dietary plan and consultation can help them improve their diet for increased treatment success.

Telehealth Services

Telehealth services help in the early identification of the deteriorating patient condition and timely intervention of the healthcare providers for elevated care effectiveness (Moulaei et al., 2023). These services provide access to health to people who are unable to pay regular hospital visits and are located in remote areas.

Support Groups

Community healthcare agencies or community groups help patients by sharing their healthcare issues and providing them with proper guidance and evidence-based practices to reduce their confusion regarding specific issues.

Education Strategies

Slide 7: Different education strategies can be implemented to improve health outcomes and increase the effectiveness of healthcare organizations. A few strategies are given below;

  • Group discussions, sessions, and awareness workshops help people in developing a better understanding of healthcare issues. 
  • A patient-centered approach should be applied by providing effective education to the single individual ensuring optimized outcomes.
  • Integration of videos, audio, graphics, and animations in education programs helps people significantly. 
  • Population diversity should be considered a major concern while developing and implementing education strategies for the population receiving care.
  • Use of healthcare information technologies for developing an enhanced understanding of the healthcare services and competencies of the professional. 

Plans to Partner with Interprofessional Team Members

Slide 8: To build strong relations between the patient and the healthcare professionals it is essential to effectively collaborate with the interprofessional team members and create an effective partnership that will lead to superior health outcomes (Carney et al., 2019). Effective team communication and cooperation can create strategies that are suitable for improved patient outcomes and high-quality health and care of the patients (Sharkiya, 2023). Their cooperation can produce advanced patient well-being along with the satisfaction of the team members due to shared opinions and high expertise. The plan to collaborate and partner with interdisciplinary teams includes the following factors;

Well-Planned Team Meetings

Team meetings should be well organized and prior informed for the better response of the team (Weller et al., 2024). Regular meetings with the mandatory presence of all the members from, different disciplines including nurses, physicians, professionals, psychiatrists, case managers, and care coordinators. The well-planned, meeting will allow the team members to share effective ideas, and opinions regarding the patient’s healthcare and suggest strategies that will impact positively on the health of the patient (Kwame & Petrucka, 2021). This can be done by effective communication and shared goals and objectives of the team members.

Common Health Plans

Health and care plans need to be shared and mutual to present results that are effective and sustainable. The shared plan will be accessible to every team member for a better understanding and opinion-building of the current health issue.

Combined Professional Development Training

Combined training of healthcare professionals allows them to create an effective partnership that will benefit the patient and the organization. It will develop a culture of involvement and fairness for the team members.

Regular Evaluation and Feedback

Feedback is necessary for the evaluation of the training programs and their effective impact on the team members. These feedbacks suggest changes that are needed to improve plans and introduce better collaboration strategies.

Shared Responsibilities and Mutual Respect

Gathering all the members on a single platform for a single purpose introduces mutual respect and gives the impression that all are valued which will have a strong positive impact on the efficiency of the healthcare providers (Samardzic et al., 2020). The roles and responsibilities were also shared and effective.

Implementation Process for Improved Care Coordination

Slide 9: Effective care coordination has great significance in generating enhanced and increased positive patient outcomes (Albertson et al., 2021). This coordination involves the team members to design and implement strategies that are most suitable for the health of the patients. Through proper involvement of all the team members, a new opinion and ideas can be introduced which will positively affect the organizational outcomes. The implementation process for the improved care coordination is given below;

  • Analysis of care coordination practices, knowledge and performance gaps, and planning for improved strategies.
  • Involvement of family members and other healthcare stakeholders to improve the decision-making process by identifying the requirements and preferences of the patient.
  • Application of healthcare technologies for the better recognition and response process to deteriorating health conditions. 
  • Standardized processes need to be implemented to ensure their effectiveness in generating better outcomes.
  • Evaluation of the effectiveness of the strategies and regular improvement will be essential for sustainable health care.

Outcomes of New Process for Improved Interprofessional Collaboration

Slide 10: Interprofessional collaboration is the collective effort of all the healthcare professionals from different departments and disciplines gathered together to generate outcomes that are beneficial for the patients, staff members, and the healthcare organization (Geese & Schmitt, 2023). The shared experiences, effective expertise, and common goals result in the development of strategies that are more likely to create an impression of unity and equity among fellow members (Kozlowski & Ilgen, 2020).

This impression of being valued motivates them to perform efficiently and proficiently for a greater and better cause of making high-quality health available to all the population. Through this collective effort, the ratio of increase in improved patient experiences and advancements in the professional and skill development of the team members will be obvious. A few outcomes of the processes for improved interprofessional collaboration are given below; 

Improved Patient Experience

The healthcare services for the patients provided by competent and effective staff will create an enhanced positive experience (Xyrichis & Rose, 2024). This effectiveness of the staff depends on the quality of collaboration between the interprofessionals.

Decrease in Medical Errors

Shared opinions and collective discussions on the healthcare condition of the patient reduce the chances of errors and also the effective training of the team members enhances their abilities to handle mistakes.

Effective Utilization of Resources

Proper discussion regarding health situations results in the effective utilization of resources as it reduces the chances of hospital readmissions and the high cost of emergency departments.

Sustained Improvement in Quality

The quality of healthcare will be enhanced due to the improved performance of the healthcare providers and shared experience of handling particular situations of patients effectively and timely.

Underlying Assumptions and Unce Coordination

Slide 11: Ethical considerations are always of great importance in the care coordination process as the consent and preferences of the patient are the priority while dealing with certain health issues (Varkey, 2020). Every individual has their own identity and they have the right to keep their information confidential and allow it only for treatment. However, it is the duty and responsibility of the care coordinators to make sure that the privacy of the patient will be maintained along with the consent of the patient for a specific treatment. 

Confidentiality and Safety

Care coordinators should make sure that the information of the patient regarding medical, social, and psychological history will be maintained and organized carefully with only access granted to the patient and the healthcare providers. 

Patient Decision-Making Authority

Healthcare decisions should be made based on the health necessities and cultural desires of the patient (Alfahmi, 2022). Patient satisfaction is one of the major concerns of the care coordinators for generating effective results. 

Accountability and Authenticity

Care coordinators will be accountable for their actions regarding the health of the patients. They should be true to their commitments and acts and should perform their duties under the guidance of the standards of healthcare laws and principles (Young & Smith, 2022). Maintaining the information accurately and creating access with the patient’s consent is necessary. They need to assess the patient’s feedback regarding the specific services provided and integrate changes in the plans according to the provided feedback.

Equity and Fairness

All patients should be treated equally with a fair level of respect and attention to give them the impression of justice and equity (Papastavrou et al., 2019). Older adults should provide enhanced and extra assistance for fast health recovery and better care outcomes.

Slide 12: Collaboration between interprofessionals is emerging as a trend due to its effectiveness and improved outcomes (Bendowska & Baum, 2023). The collaboration of teams in evidence-based practices produces results that are patient-centered and ensures sustainable quality, and health for all the population. Advanced education and training of the population receiving health care is the need to prove effectiveness of the interprofessional collaboration.

Educational strategies of involving patients in decision-making along with implementing self-management plans are essential to increase the rate of positive outcomes. The proficient relationship between the team members results in sustainable health outcomes by highlighting the importance of partnership between the team members (Varkey, 2020). These collaborations generate patient-satisfied outcomes by carefully considering the ethics involved in patient treatment.
