Change Strategy and Implementation

The management of the health situation of the patient by stabilizing the acute health condition of the patient is necessary as it will reduce health disparities and increase the effectiveness of the patient experience (Rodziewicz et al., 2024). In handling health situations with proficiency it is necessary to design and implement strategies that are advanced and evidence-based in providing effective health and care to patients. Acute renal failure is a situation that occurs during the increased level of diabetes, ineffective monitoring of blood glucose levels, poor insulin consideration along managing the ineffective diabetic diet. This condition can be improved through the use of change strategies that will ensure the quality of treatment provided to the patient along with the collaboration of interprofessional team members. 

The change strategies are best for monitoring the health situations of the patient with great care using the data based on evidence of effective care coordination (Nilsen et al., 2020). These strategies should be focused on improving the level of blood glucose and overall health of the patient by effectively insulin requirements and diabetes levels. These change strategies should play a significant role in providing enhanced knowledge to the patient regarding the health situation so that they can be able to implement self-management approaches with great care and safety. 

Data Table Reflecting Current and Desired Outcomes

The data table is designed to understand the treatment provided to the patient based on the current health state of the patient and identify the desired state by monitoring the medical history of the patient (Ferreira et al.). All these analyses will be effective in implementing a new strategy that will introduce a change in the health management of the patient. The effective implementation of a change strategy always involves the patient’s needs and the collaboration of interprofessional teams to highlight the areas of concern and identify gaps in the care provided to the patient for better patient experience.

Areas of Uncertainty

There is uncertainty regarding the effective monitoring of blood glucose levels after discharge. Acute renal failure has been stabilized but it is uncertain whether it is fully recovered or not. Regular monitoring of fluid levels and reducing the practice that creates hindrances in maintaining the appropriate level of fluid. There is no certain information regarding the blood pressure of the patient in the case study.

Proposing Change Strategies

Acute renal failure situations can be handled by implementing strategies that effectively handle all the identified issues related to the failure of the kidney’s proper functioning (Goyal et al., 2023). These strategies need to identify ways that are effective and efficient in handling the patient situation with the complete consideration of the needs and preferences of the patient. Below are some change strategies to proficiently handle the situation of the patient;

Controlling Blood Glucose Level

Regular and timely monitoring of blood glucose levels and providing timely reminders to the patients regarding any medical application for the management of health situations is necessary (Weinstock et al., 2020). The change strategy is based on the consideration of an effective diabetic meal plan for the patient that would help them effectively monitor the renal situation and diabetic management. Effective education should be provided to the patients to understand the need for proficient monitoring of the health situation. The patient might fail to properly follow the diet plan or may forget the time of their medications. This problem can be resolved by allowing real-time communication between the patient and the dietitians and providing regular reminders to the patient through the use of technology-based applications. 

Renal Function Management

Regular home monitoring of fluid level to prevent excessive fluid and manage medications according to the health condition of the patient regarding acute renal failure. Provide education to the patient regarding the exercises that would help maintain the health of the body and the kidney functioning improvement (Arazi et al., 2022). Patients might feel difficulty in managing and monitoring the fluid levels which could worsen the renal condition. This problem can be resolved by paying regular home visits to the patient along with education regarding the maintenance of fluid levels. 

Controlling Blood Pressure

Provide education to the patient regarding the monitoring of blood pressure at home and managing hypertension medication properly under the instructional guidance provided by the health care practitioner (Dymek et al., 2019). Highlight the importance of a diet containing a low quantity of sodium which might be challenging for the patient. Patients can often forget to monitor their blood glucose levels which can be identified by conducting regular follow-ups from the patient. Provdie a proper dietary plan to the patient that is in consideration of low sodium and diabetic effective to reduce renal failure. 

Dietary Improvements

Provide an effective dietary plan to the patient by consulting with dietitians who will analyze the history and meals of the patient and suggest changes and improvements according to the findings (Sikand et al., 2023). Consult with the patient’s family to inform them clearly and accurately regarding the need for an effective diet plan and meals for the patient’s better health condition. Patients might find it challenging to properly follow the diet plan or acquire appropriate support from family. This can be resolved by using community resources that can provide proper easy-to-ready meals for effective management of diabetes and blood pressure.

Justification of Change Strategies

Change strategies are based on evidence-based practices that play a huge role in ensuring the effectiveness of the treatment provided to the patient to stabilize the acute health condition and present ideas that are best suitable for the deteriorating patient condition (Roisin et al., 2024). These change strategies help in the identification of ways that can be implemented to effectively monitor and analyze the health situation of the patients and provdie ways that are advanced and effective in managing the health condition of the patient. The telehealth strategy or timely monitoring of the blood glucose level will improve the diabetic levels along with reducing the risk of renal failure or hospital readmission. Continuous glucose monitoring is the machine that will provide real-time data regarding the level of blood glucose for early interventions. The tracking of the fluid levels and regular follow-ups from the nursing staff ensure the maintenance of fluids in the body and validate the decrease in the risk of renal failure. Moreover, effective monitoring of blood pressure by introducing a diet that is sodium-free or containing low sodium reflects the importance of an effective dietary plan in managing the health situation of the patient with great care and efficiency (Byung et al., 2024). The diet plan is necessary to control diabetes and blood pressure therefore involvement of dietitians in recommending the best management plan is of key importance. 

Change Strategies for Quality Improvement

Change strategies play a significant role in ensuring the quality of Health provided to the patient by considering the acute health condition of the patient and the interventions that are most necessary to improve the health condition effectively. The timely monitoring of blood glucose levels ensures the safety of the patient by decreasing the risk of exacerbating renal failure and controlling the measurement of diabetes (Kumar et al., 2023). The care provided to the patient considers and identifies the cultural background along with the resources available to provide efficient care to the patient. By recognizing and responding to the early signs of deteriorating conditions patients will be able to be saved from great health damage thus ensuring that the safety of the patient is maintained by regular monitoring of the health situation. Providing the facility with home interventions and health services along with regular follow-ups about the patient’s condition ensures the care is centered on the patient. 

The monitoring of blood pressure through the use of an apparatus allows the patient to recognize and respond to the hypertension situation early to reduce the risk of health disparities thus ensuring the safety of the patient (Khoong et al., 2022). Moreover, the education and plan provdie to the patient regarding the effective monitoring of the blood ensures the quality of care the patient needs.  Providing the dietary plan to the patient ensures that the health effectivity will be checked and the safety regarding the exacerbation of the situations is maintained by providing an early response to the health condition. Proving the dietary plan that is prepared by considering the cultural and traditional factors of the dietary needs of the patient for effective health response. 

Assumptions Identification

It is assumed that the patient has the necessary knowledge needed to monitor blood glucose levels and dietary needs. The structure and environment of the home are adjustable to the technology-based interventions is another assumption associated with the effective monitoring of blood glucose levels (Mansour et al., 2024). It is an assumption that the patient is regularly keeping track of levels of the fluid. It is assumed that the patient has the capability and resources to monitor the level of blood pressure regularly and the telehealth facility will be available for early intervention. It is assumed that the patient has access to the dietary food resources and the patient is confident to stick to the diet plan. 

Change Strategies Utilizing Interprofessional Considerations

Interprofessional collaboration holds great significance in validating the quality of care and safety provided to the patient by integrating their expertise and evidence-based strategy for the improved patient experience and enhanced outcome of the health of patients. The effective handling of acute renal conditions requires the collaboration of interprofessional teams including dietitians. pharmacists, endocrinologists, nephrologists, home health nurses, and primary care providers (Murdeshwar & Anjum, 2023). They all play a significant role in managing the improving the health of the patient by ensuring a high standard quality of care provided to the patient. They all together communicate and coordinate to invent, design, and implement strategies that are most effective in providing care to the patient based on the cultural and traditional patient preferences and enhanced health outcomes regarding the acute condition of the patient. 

The implementation of care plans and diet plans regarding the management of the levels of blood glucose and the sodium percentage in the food to ensure that the risk of renal failure is decreased and health outcomes have been improved. The effective monitoring of the level of fluid to prevent overloading and manage health quality is also validated by the collaboration of the interprofessionals and their expertise (Siew et al., 2024). The integration of telehealth service in the care coordination of the patient involves technical experts who share their opinions regarding the principles and techniques of the remote monitoring and processing of telehealth machines. Knowledge obtained from the collaboration allows the effective performance of the team members and the patients in early recognition of the challenging situations and timely intervention of the professional to stabilize the condition.

The health and safety of the patient is the priority of every healthcare provider due to which the change strategies are designed and implemented so that effective treatment should be provided to the patient. These change strategies identify ways that can be used to successfully understand the health condition of the patient and apply the knowledge that is best suitable to improve the health condition of the patient (Rodziewicz et al., 2024). These strategies ensure the quality of the treatment provided to the patient through the intervention of technology in the treatment plan and the provision of effective health management plans. Moreover, the collaboration of interprofessional ensures that all the aspects of the health of patients are considered and coordinated care is provided to the patients depending on the needs and preferences of the patient for improved patient experience and effective patient outcomes.
